For testing, allow using wxAuiMDI{Parent,Child}Frame in the sample.
This change also shows that the AUI classes are now sufficiently
compatible with the standard ones for switching to them to be almost
trivial.
Cocoa has been the default toolkit in wxWidgets for a long time. There is really no good reason to use Carbon in 2016 and this removes a lot of unused and unmaintained code.
This fixes compilation in C++11 mode: "override" results in an error on
non-Mac platforms because this method doesn't exist there and so can't be
overridden.

Don't support this compiler any more, this allows to get rid of tons of
MSVC6-specific workarounds, in particular we can now use Bind() and natural
template functions calls in the library code.
Also remove MSVC6 project and solution files and don't generate them when
bakefile_gen is ran any more (removing the remaining occurrences of msvc6prj
from the bakefiles results in weird bake-time errors, so it's simpler to just
leave them there).

This keyword is not expanded by Git which means it's not replaced with the
correct revision value in the releases made using git-based scripts and it's
confusing to have lines with unexpanded "$Id$" in the released files. As
expanding them with Git is not that simple (it could be done with git archive
and export-subst attribute) and there are not many benefits in having them in
the first place, just remove all these lines.
If nothing else, this will make an eventual transition to Git simpler.

Add a special symbol which is defined only if the icons and other images (e.g.
cursor) are in the separate resource files and don't need to be embedded as
XPMs in the main program.
This makes the checks more clear and more customizable as it's enough to
change wxHAS_IMAGES_IN_RESOURCES definition instead of changing many platform
checks.

Show how the file history can be saved to and restored from wxConfig.
This makes it much easier to test file history related stuff as the history
doesn't need to be recreated during every sample run.
It is also closer to what real applications using docview framework do as the
file history is relatively useless if it's not saved.

It is definitely not necessary to call SetTopWindow() when there is only a
single top level window and it is arguable whether it's useful to do it even
when there are many of them so don't encourage its use in the documentation
and also remove all its occurrences from the samples.

Remove unnecessary, never used methods.
Don't use pointers when objects or references can be used more safely.
Don't name classes which are not part of wx with "wx" prefix to avoid
confusing people.
Don't define empty unnecessary event tables.
Prefer wxScrolledWindow::SetVirtualSize() to SetScrollbars().
Stop using "protected" when "private" should be used.

wxDocument::Revert() method existed but didn't do anything, provide a
default implementation for it. Also document it and show it in the sample.
