Ensure that all derived classes have these functions and let them to
avoid defining them if they can just use the default implementation,
which was the case for most ports.
Also move m_bitmap to the base class from the derived ones.
No real changes.
wxDEPRECATED must be used for the entire declaration to really work,
but this wasn't the case for a few functions (and one typedef) guarded
by WXWIN_COMPATIBILITY_2_8, resulting in gcc -Wattributes warning about
the attribute being ignored and the declaration not being really marked
as deprecated.
Fix this by applying the deprecation macro to the entire declaration,
which works well for gcc and should hopefully work for MSVC too.
These accelerators are not shown in wxMenuItem label, but still will
work.
Implement support for them in all major ports and XRC.

Define it in wx/defs.h directly instead of conditionally including
wx/windowid.h from there just in order to get this type definition.
This has several advantages:
- wxWindowIDRef is not defined in (maybe) non-GUI code including
wx/defs.h, as it should be the case for this class defined in
core library only.
- wx/windowid.h becomes a normal header, including wx/defs.h as
(almost) all the other ones instead of being exceptional.
- wx/windowid.h doesn't need to be included by wx/utils.h at all
just to get wxWindowID definition.
Closes https://github.com/wxWidgets/wxWidgets/pull/1850
This merges in the latest sources from GSoC 2014 wxQt project with just a few
minor corrections, mostly undoing wrong changes to common files in that branch
(results of a previous bad merge?) and getting rid of whitespace-only changes.
Also remove debug logging from wxGrid.

This should have been part of r76735: in addition to removing the old Cocoa
port headers, also remove the references to them from the common include
files.

This port is not used and is not being worked on, so remove it to reduce the
amount of the code which needs to be updated for every global change.
Also remove tests for VisualAge compiler which isn't used since ages.

This keyword is not expanded by Git which means it's not replaced with the
correct revision value in the releases made using git-based scripts and it's
confusing to have lines with unexpanded "$Id$" in the released files. As
expanding them with Git is not that simple (it could be done with git archive
and export-subst attribute) and there are not many benefits in having them in
the first place, just remove all these lines.
If nothing else, this will make an eventual transition to Git simpler.

The latter will be convenient to use in the upcoming changes to wxOSX radio
items management code and add the former for the symmetry.

The platform targeted by this port doesn't exist any more and the port never
achieved really working state so remove the code to avoid having to maintain
it.

Intel compiler gives warnings if the static keyword is not at the beginning of
the declaration and this happened when using it inside wxDEPRECATED() and
related macros because the declaration started with __declspec(deprecated) or
__attribute__((deprecated)) instead.
To avoid this problem, simply move "static" outside the macro.
